Opioid Analgesics Association with Serotonin Syndrome in Patients Taking Antidepressant Medications

Antidepressant use in the United States has increased nearly 65% over a 15-year timeframe (from 1999-2014) (Centers for Disease Control and Prevention [CDC], 2017). This rise in antidepressant use increases the risk of serotonin syndrome for patients in both the perioperative setting and in pain clinics where the administration, and/or prescribing, of opioid analgesics is common practice. This online education module is specifically intended for Certified Registered Nurse Anesthetists (CRNAs) who work in the perioperative setting, pain clinics, or other areas where opioid analgesics are commonly administered or prescribed but can be useful to other healthcare professionals as well. When serotonin syndrome is misdiagnosed or not recognized and treated in a timely manner, it can rapidly become a life-threatening condition.

Upon completion of this course, program participants will be able to:

  1. Defining and understand the causes of serotonin syndrome.
  2. Determining the impact healthcare professionals can have on improving the gap in knowledge regarding serotonin syndrome development in patients taking antidepressant medications presenting to pain clinics or for surgery where opioid analgesics are prescribed and administered.
  3. Reviewing the signs and symptoms of a patient experiencing serotonin syndrome.
  4. Discussing patient populations who are at an increased risk of developing serotonin syndrome.
  5. Specifying serotonergic antidepressants and opioid analgesics medications associated with serotonin syndrome as well as other pro-serotonergic drugs and their association with serotonin syndrome.
  6. Selecting opioid analgesics that have not been associated with serotonin syndrome to assist with pain management of patients at an increased risk of developing serotonin syndrome.
  7. Utilizing the Hunter Serotonin Toxicity Criteria for diagnosing serotonin syndrome.
  8. Analyzing the necessary treatment plan for patinets presenting with mild to severe signs and symptoms of serotonin syndrome.
  9. Differentiating between conditions that closely resemble serotonin syndrome.

Steven Fowler, DNP, CRNA

Dr. Fowler is a faculty member and associate professor at AdventHealth University’s Nurse Anesthesia Department. He has over 28 years of experience as a certified registered nurse anesthetist (CRNA) and over 8 years teaching clinically. He received his Doctor of Nursing Practice (DNP) from Wilkes University in 2013, Master of Science (MS) in Nurse Anesthesia from Mercy College in Detroit in 1990, and his Bachelor of Science in Nursing (BSN) from the University of Florida in 1988. Dr. Fowler has been teaching full-time at AdventHealth University since 2015 which includes classroom lectures, simulation training, and clinical precepting. In 1991 Dr. Fowler received the Michigan Association of Nurse Anesthetists Master Thesis Award and has been published in the Journal of Nursing Research and Practice, 2018.

Joselline Garcia, BSN, RN, CCRN

Joselline Garcia is a graduate student in the Nurse Anesthesia Program at AdventHealth University. She obtained a Bachelor of Science degree in nursing in 2013. She focused her career within various hospitals and worked in the cardiovascular, medical, surgical, and, neurovascular intensive care unit. She is certified in Pediatric Advanced Life Support, Advanced Life Support, Basic Life Support, and is a registered Critical Care Nurse (CCRN). She is also a current member of the American Association of Nurse Anesthetists and Florida Association of Nurse Anesthetists.

Jennifer Huddlestun, BSN, RN, CCRN

Jennifer Huddlestun is a graduate student in the Nurse Anesthesia Program at AdventHealth University. Prior to pursuing her graduate degree, she earned a Bachelor of Science degree in nursing from Florida State University in Tallahassee, FL. She has over five years of experience as a critical care nurse in the cardiovascular, surgical, and trauma intensive care units. She is a current member of the American Association of Nurse Anesthetists and Florida Association of Nurse Anesthetists.

Revisioning Author: M. Julie Talmadge, DNP, APRN, FNP-BC, APPD

Dr. Talmadge is a family nurse practitioner with over 18 years of experience as a nurse practitioner. Her most recent clinical experience was in acute care working with a surgical residency program in Orlando, Florida. In 2013 she was awarded her Doctorate of Nursing Practice from the University of Florida in Gainesville. She currently is the Accredited Provider Program Director with echelon, Continuing Education and Professional Development, a Division of AdventHealth University. Dr. Talmadge is a member of the Central Florida Advanced Nursing Practice Council. She served on the Florida Board of Nursing, as appointed by the Governor (term 2018 - 2021). She has been adjunct faculty at the University of Central Florida and has precepted graduate nurse practitioner students.
