Prevention of Medical Errors

Prevention of Medical Errors is a 2-hour course fulfilling a requirement for license renewal focusing on patient safety, root cause analysis, and medical error prevention. Discussed will be the importance of replacing a culture of blame with a culture supporting honest reporting, thereby building a successful plan targeting known problems.

Upon successful completion of this course, the participant will be able to:
  1. Describe the magnitude of medical errors and the effect on patient safety.
  2. Describe processes to improve patient outcomes.
  3. Recognize error prone situation/processes.
  4. Identify factors that impact the occurrence of errors.
  5. Define the process and benefit of multi-causal analysis (root causes).
  6. Describe his/her responsibility for reporting medical errors as related to the policies and procedures of his/her particular facility.
  7. Identify safety needs of special populations.
  8. Discuss educational needs of patients/families.
  9. Discuss the importance of public education regarding medical errors.

Guy Flynt, BS, CIC, AU

Guy has over 20 years in the risk management and insurance industry. He received a Bachelors of Science degree from Columbia Union College and is currently working toward a degree in law. He has experience in Hospital Professional Liability as well as Worker's Compensation and Managed Care. He has served as Chair of the Quality Assurance Committee for his organization and is certified as a Health Care Risk Manager through University of Central Florida.

Revisioning Author: P. Gage Gwyn, Ph.D., APRN-BC, CNS, OCN, FCN

Priscilla "Gage" Gwyn is an adult oncology nurse practitioner at the Ambulatory Infusion Center at AdventHealth in Greater Orlando (formerly Florida Hospital).  Dr. Gwyn received her Bachelor of Science in nursing from the SUNY Brockport College of Nursing in New York and her Master of Science in nursing with Adult Nurse Practitioner and Clinical Nurse Specialist certificates from Duke University College of Nursing in Durham, North Carolina.  She then received her PhD in Nursing from the Barry University, Division of Nursing, in Miami Shores, Florida.  She has lectured on the care of patients with hematologic malignancies and has initiated, coordinated, and facilitated monthly support groups at several practice locations in Florida.  She is nationally published and is a current member of the Oncology Nursing Society, Society of NeuroOncology, and Greater Orlando Hematology Oncology Physician Extenders.

Revisioning Author: Lynn Rowe, PhD, RN

Dr. Rowe has been an adjunct faculty member with AdventHealth University (AHU) since 1997 and fulltime faculty member since January 2018. Her degrees include a BSN-Florida State University, MSN-University of Phoenix (Education, and Leadership) and PhD for University of Arizona (Biological Injury). Her research interests include issues in complications of transplantation, and multiple other topics surrounding the practice of nursing. Dr. Rowe has over 30 years of clinical experience including cardiovascular progressive care, multisystem intensive care unit and liver, pancreas, and kidney transplantation. 12 years as a unit/system educator for Neurointensive care and Multisystem ICU and 5 years in support in the nursing Research department. Dr. Rowe continues to support nursing practice through collaborative activities with various hospital campuses and serves as the chair of the Scientific review committee to review the collaborative efforts of university faculty.

Merrie Lynn Williams, BSN, RN

Lynette Woide received her diploma of nursing from Fairview General Hospital School of Nursing.  She furthered her studies and obtained her BSN and MSN from the University of Phoenix.  Her professional career has focused on oncology.
